About us
DigiCare4You is an innovative, easy-to-implement, low-cost, and sustainable solution which will help build healthcare services’ capacity and strengthen countries’ primary care systems as well as address the specific needs and preferences of people at risk of T2D and/or hypertension, as they develop healthy habits and engage in activities supporting their well-being. Aiming to be a fully scalable and transferable solution, a roadmap will be developed to help policymakers assess the feasibility and effectiveness of the DigiCare4You solution in their regions/countries and promote people-centred care and integrated health services in Europe and beyond.

🧠 On the occasion of #WorldBrainDay 2025, we highlight the importance of preventing and managing diabetes to reduce the risk of brain-related diseases. People living with diabetes are 1.5 to 2 times more likely to experience a #stroke than those without diabetes. Type 2 diabetes (#T2D) has also been linked to neuropsychological dysfunction, dementia, Alzheimer’s disease and Parkinson’s disease. This strong connection underscores the need to prioritise both the prevention of diabetes and the effective management of the condition to reduce the risk of complications affecting the brain. DigiCare4You aims to strengthen the early prevention and management of #type2diabetes and #hypertension.
