This role includes one evening until 7:30 per week and participates in a telephonic call and half day Saturday rotations .

The Integrated Care Nurse Practitioner / Advanced Practice Registered Nurse is delegated the authority by their supervising physician to perform physical examinations and order diagnostic testing, including but not limited to laboratory tests, diagnostic imaging tests; to make clinical assessments, and implement a plan of treatment, to include, but not limited to, the prescribing of medication as so granted by Connecticut statutes. It is understood that the authority to perform these delegated tasks outlined above comes from the licensed supervising physicians within Optimus who shall provide oversight over the clinical activities of those individuals under their authority. The Integrated Care Nurse Practitioner / Advanced Practice Registered Nurse may serve as primary care provider within certain departments, and as such, may carry their own patient panels.
Functions & Responsibilities
Obtain complete medical history and physical data on patients.
Interpret and integrate data to determine appropriate diagnostic and therapeutic procedures as needed.
Perform complete physical exam and records findings in electronic medical record. Collaborates with physicians as needed in managing acute and long-term medical needs of the patients.
Order appropriate laboratory and diagnostic procedures.
Synthesize data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
Administer medications and injections as needed.
Interview and advise patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.
Instruct patient and family regarding medications and treatment instructions. Provides patient education.
Maintain and review patients’ records, charts, and other pertinent information. Posts tests and examination results.
Triage patient telephone calls and provides consultation, and appropriately refer patients to additional needed services.
Collaborate with other practitioners and departments within and external to Optimus to facilitate warm handoffs and multidisciplinary care.
Attend Integrated Care Case Review and other relevant meetings as required; participate in committees as requested.
Participate in professional development activities and maintains professional affiliations.
Maintain patient confidentiality.
Perform other related work as required.
This position includes participation in telephonic call and Saturday rotations.
EDUCATION: Graduate of an accredited school of nursing and successful completion of an approved Nurse Practioner program.
EXPERIENCE: One to three years’ experience in the area of pediatrics and / or adult medicine.
LANGUAGE SKILLS: Bi-lingual skills in Spanish preferred
LICENSURE / CERTIFICATION: Licensed to practice as a Nurse Practitioner / Advanced Practice Registered Nurse in the State of Connecticut. Board-certified APRN
