- 4. + Signing Up Fill out the form at vaccinefinder.org/admin/signup Wait until you receive an email confirming your account has been approved to proceed (usually within 24 hrs).

- 9. + Submitting Data Uploading Individual Location or Multiple Locations
- 10. + Uploading an Individual Location Click the “Add Provider” tab in the upper right corner.
- 11. + Uploading an Individual Location Fill out the information in the Individual Provider Entry Form
- 12. + Uploading an Individual Location Enter any information on age restrictions on vaccination, insurance accepted, and clinic hours. Select your provider type (clinic, pharmacy, health department, etc)
- 13. + Uploading an Individual Location Enter date ranges you want your flu vaccine clinics/locations to appear on the map. You will NOT show up on the map if you leave the date field blank! Check off the vaccines you offer and include how you’d like to display pricing.
- 14. + Uploading an Individual Location Repeat for the Routine Vaccines. Date ranges do not apply to routine vaccines.
- 15. + Uploading an Individual Location Click Submit at the bottom of the page.
- 16. + Uploading Multiple Providers To upload a spreadsheet, download a sample spreadsheet template from the Add Provider page.
- 17. + Uploading Multiple Providers Fill out the template using Excel or similar software.
- 18. + Uploading Multiple Providers There are a number of formatting guidelines. To access the guidelines document, go to the Help page.
- 19. + Formatting Rules Here, you can download a pdf of the formatting guidelines.
- 20. + Formatting Rules (abridged) Rule 1: Your spreadsheet must be saved as a comma separated values document (.csv) Rule 2: Certain fields are required. Rule 3: Some fields are a simple yes/no answer. Rule 4: The address fields have special rules. Rule 5: Other miscellaneous requirements.
- 21. + Uploading Multiple Providers Once your spreadsheet is ready, return to the Add Provider page and click “Choose File”
- 22. + Uploading Multiple Providers Select your saved .csv file.
- 23. + Uploading Multiple Providers Click Submit.
- 24. + Uploading Multiple Providers The system will take a few moments to geolocate your clinic/pharmacy locations. This may take a while if you have many locations. Do NOT close the window or tab while the file is uploading.
- 25. + Uploading Multiple Providers If there are any errors with your spreadsheet, you will see error messages with instructions on what to correct. If all goes well, you will see this “successfully uploaded” message.
- 26. + Viewing & Editing Data
- 27. + Provider List Tab The Provider List tab shows a table of your locations. You can filter to find a specific location, or click on column headers to reorder the list.
- 28. + Edit an Individual Location To edit a single location, click on its name in the Provider List table.
- 29. + Edit an Individual Location Edit any information in the Entry Form. Be sure to click Submit at the bottom of the page!
- 30. + Edit Multiple Locations First, download a spreadsheet of your existing data, found on the Provider List and Add Provider tabs. This helps avoid any duplication of locations!
- 31. + Edit Multiple Locations Make any changes in your downloaded .csv file. Be sure to follow the guidelines found in the Formatting Rules document.
- 32. + Edit Multiple Locations Return to the Add Provider tab, and upload your updated spreadsheet.
