ADD QUESTIONS TO A
SURVEY
Facilitator: Matthew Pittman
Revised: 01/27/2013
Question Types
• SurveyMonkey allows the creation of many types of
questions including text boxcomments, multiple choice,
rating scales, and many others. We will cover multiple
choice and rating scales in this presentation.

• SurveyMonkey also contains a database of questions that
the user can pick from as well. This presentation will
show the user how to create new questions from scratch
but in many cases questions may already be available
that will pop up as you type the question text that may fit
your needs. Using these will help speed up the process
 of creating your survey.
MULTIPLE CHOICE
QUESTIONS
(Only One Answer)
Step One
• From the Design Survey tab of a survey click the
+ Add Question button on any page of the
 survey
Step Two
• Type your question
 text and then select
 the Multiple Choice
 (Only One Answer)
 question type from the
 dropdown. In the
 Answer Options area
 type your answers
 (one per line). Click
 Save & Close when
 you are done.
RATING SCALE
Step One
• From the Design Survey tab of a survey click the
+ Add Question button on any page of the
 survey
Step Two
• Type your question
 text and then select
 the Rating Scale
 question type from the
 dropdown. In the
 Rating Scale area
 select the number of
 ratings, their weight,
 and the labels to
 display. Click Save &
 Close when you are
 done.
You Are Done!
• At this point you have
 learned to add two
 different types of questions
 to a survey. Other
 question types are just as
 easy and you should try
 others that interest you.
 Your survey is now ready
 to be distributed to your
 respondents.
