COMMISSIONING INDUCTION REQUIREMENT After completion of KJP HSES Mandatory Induction, those who work at commissioning controlled area are required to attend the commissioning induction. COMMISSIONING PERMIT TYPES AND USES: • All work in Commissioning Designated Areas or Live System will require a Commissioning Work Permit and is to be displayed at the work location • Permits will be issued from Permit Issuing Station on site (TBA) TANGGUH LNG PROJECT HSE TRAINING DEPARTMENT KBR

PERMIT TO WORK SYSTEM There are three types of Permit To Work to be used during commissioning activities: Responsibility of the Performing Authority Issued & approved by KJP Commissioning team Valid for 12 hrs shift and may be extended for another 12 hrs shift (max 7 days) KBR TANGGUH LNG PROJECT HSE TRAINING DEPARTMENT

PERMIT TO WORK SYSTEM Hot Works: Required for all activities in an operating area, where the possibility of having an ignition source exist such as: Instrumentation / Electrical work on operating equipment Opening of live electrical cabinets / switchgear Vehicle Entry Use of heat source / spark inducer, open flame, and or grinding A GAS TEST WILL ALWAYS BE REQUIRED FOR ANY HOT WORKS (LEL 0%) KBR TANGGUH LNG PROJECT HSE TRAINING DEPARTMENT

CONFINED SPACES ENTRY: Confined space entry is not only refers to complete body entry but also partial entry of your body such as head, hands, feet into a confined space. ENERGY ISOLATION (LOTO) To ensure personnel are notified that safety or emergency system are to be bypassed This permit is valid for 12 hrs or an operations shift change, whichever come first. COMMISSIONING CLEANING METHODS The following cleaning methods will be used on KJP LNG Tangguh Project: Air Blowing Steam blowing Mechanical Cleaning Chemical Cleaning (Boilers, Furnace, etc) Water Flushing There are hazards associated with all cleaning methods. - 29. 29 BPMIGAS TANGGUH LNG BP Berau Ltd. GASEOUS HYDROCARBON Before hydrocarbons are introduced into any system, the 02 content must be reduces to avoid an explosive atmosphere. Prior introduction of gaseous hydrocarbons in to the plant, KJP Commissioning shall: Ensure the system is tightness tested Ensure the system is N2 purged until 02 content is < 2% Ensure the system is kept under slight N2 pressure until hydrocarbons introduced KBR TANGGUH LNG PROJECT HSE TRAINING DEPARTMENT
