This document discusses common problems that occur with Hazard and Operability (HAZOP) studies. It notes that while guidelines for best practices in HAZOP studies exist, in practice it can be difficult to fully follow best practices. Some common issues include using HAZOP by difference for repetitive designs without fully considering differences in process conditions, and reluctance of vendors to acknowledge potential issues with their package designs. Additionally, there has been a trend to reduce the scope of HAZOPs to only safety and environmental issues, excluding operability and reliability. With financial pressures, these problems are expected to increase as projects face pressure to reduce costs and scope. The document explores some potential solutions to these common HAZOP challenges.
