The roll-out of the xpert mtb/rif diagnostic technology for tuberculosis has significantly improved detection rates, especially for multidrug-resistant TB, across 122 countries since 2011. However, challenges such as high costs, inadequate training and quality assurance, and weak health systems have limited its impact on patient outcomes. The authors advocate for a comprehensive approach to diagnostics implementation, emphasizing greater stakeholder engagement and systems strengthening to enhance future effectiveness.
