ADDITIONAL CLARIFICATION ON THE RULES AND
REGULATIONS ON THE EVALUATION OF MASTER TEACHER
DOCUMENTS PURSUANT TO DECS ORDER No. 10 and 29,s.
1979,s. 1997
1. There will be a yearly evaluation of teacher candidates
for Mater Teachers in the Division with following timetable

District Level-----
Division Level----
Release of Ranklist
2. All Master teacher candidates who qualified but were not given items
should be included in the evaluation during the succeeding school year. They
should be made to update their papers;
3. In case wher vacancy occurs before the release of the new ranklist by the
Division Office the last year’s ranklist shall be used;
4. Only demonstration teachingdone within five years shall be given credit for
Master Teacher 1, whether the demonstration is school/district/division
/regional level.However FOR Master Teacher 2, ONLY DEMONSTRATION
TEACHING DONE DURING THE YEAR,THE TIME THE CANDIDATE WAS
A MASTER TEACHER 1 will be given credit,provided the demonstration
teaching was on the division/regional/national level;
5.Only accomplishment done by the teacher within the last 5 years shall be
given credit under Leadership,Potential and Accomplishment For Master
Teacher 2, only accomplishments done during the times the teacher
candidate was a Master Teacher 1 shall be given credit;
6.Item A UNDER Leadership ,Potentials and Accomplishments , particularly sub
items 1,2,3 and 4 only those undertaken by the individual teacher on her own shall
be given credit;
7. For item G, In case where several repeated meritorious achievements where
made by the teacher candidate at any level ,the said teacher will be given
corresponding credits for each achievement , the total number of which should not
exceed the required maximum number of points;
8. Any supporting documents whish are photo-copied ,they must be certified as
true copy the original by the issuing officer,unless decided by the Division or any
alternative decision on said documents
9. Only the candidates who qualified should be in the ranklist
10. If there is only one candidate who qualified, a remark stating that there is only
one candidate who qualified among the total number of candidates who submitted
their papers
11 All documents must be numbered consecutively and initialed by the members
of the ranking committee;
12. All members must sign the encoded ranklist( page by page) in a number of
copies required ,duly signed by the PSDS/school head
13. It is the responsibilty of the School Head /PSDS to disseminate the results of
the ranking only when the SDS has signed the said document
14. When the documents , with the ranking, are in the division ,submitted through
a transmittal letter, and has been stamped received,no additional documents shall
be accepted
I. BASIC REQUIREMENTS DOCUMENTS/SUPPORTING PAPERS
1. Permanent Teacher 1.1 One certified xerox copy of CSC/PBET
Or PRC/Let Rating/R.A. 4670
1.2 One certified xerox copy of Appointment
Paper/Service Records/Certification Signed by
Superintendent or Administrative Officer.
2. Bachelor’s Degree for Teachers or its Equivalent
as provided in the Magna Carta for teachers
MT I - - - - - - 18 MA UNITS
MT II - - - - - CAR
MT III - - - - - MA or 20 yrs. Experience + 20 MA Units
MT IV - - - - - MA, MAT,. MED, Ph. D. Ed. D.
2.1 One Certified xerox copy of transcript of record
with statement of graduation and/or special order
for graduation if graduate of a private school
/recognized by the CHED/Government
2.2 Certification from the Dean of Graduate
School/Transcript of Records
(Inclosure No. 2 to Regional Memorandum No. 157, s. 1998)
CONSOLIDATED EVALUATION GUIDELINES FOR MASTER TEACHER POSITION
REFERENCES: E. O. No. 500 of the President dated March 21, 1978
DECS Order 10, s. 1997
DECS Order No. 29, s. 1979
NBC No. 303 dated April 10, 1979
Regional Memorandum No. 157, s. 1989
DECS Order No. 57, s. 1997
3. Very Satisfactory Rating for the last two years (at
least 8.3 Performance Rating)
3.1 One Certifdied Xerox Copy Performance Rating
Sheets for the last two/three years, duly signed by
the Rater, Ratee and duly approved by the SDS or
his duly authorized representative
4. Experience Requirements
MT I - - - - 3 yrs. As a Teacher
MT II - - - 1 yr. as MT I
MT III - - - 1 yr. as MT II
MT IV - - - 1 yr. as MT III
4.1 One copy of certification by the Principal/ District
Head
4.2 One certified xerox copy/photocopy of the recent
Service Record
5. Relevant Training Requirement
MT I - - - - 8 hours
MT II - - - 16 hours
MT III - - - 24 hours
MT IV - - - 32 hours
5.1 One certified xerox copy of Certificate of
Participation
6. Teaching Load Requirements at least 270 minutes
teaching load for all MT positions, if not adviser plus
other ancillary load/activities
6.1 One copy of teacher program duly signed by
Principal/District Head (All photocopies/xerox copies
should be duly authenticated/certified(recentyear)
REQUIREMENTS FOR DEMONSTRATION TEACHING (10 PTS)
1. Certification (1 copy from concerned school authorities indicating the rating among other things.
2. Copy of the lesson plan approved by the School Head
MT I – Certification from Principal/District Head (School Level)
MT II - Certification from the EPS I or EPSII/ASDS (Division Level)
MT III & IV – Certification from an ES II or the EED Chief (Regional Level/above)
For qualifying purposes, the following total number of points for the following positions shall
be observed:
MT I - - - - - at least 25 points
MT II - - - - at least 30 points
MT III - - - - at least 45 points
MT IV - - - - at least 50 points
II. LEADERSHIP POTENTIALS ACCOMPLISHMENT DOCUMENTS/SUPPORTING PAPERS REQUIRED
A. Introduced the following for use in school or district (20 points for
any one of the items)
1. Curriculum or Instruction Materials (CIM)
1.1 one copy of the certification by the District Supervisor that the
CIM was originally developed by the applicant
1.2 one copy of the Division Bulletin/Circular enjoining that said
materials must be used;
1.3 one copy of the Instructional/Curriculum Materials, or a Picture
of the Instructional Materials with a write-up, be it a hardware IM.
1.4 One copy of the certificate of Award/Recognition duly signed
by the Schools Division Superintendent.
2. Effective Teaching Techniques/Strategies 2.1 One copy of Division Memorandum/Letter stating that the
teaching Techniques/Strategies developed be used in the Field.
2.2 One copy of the certification by the Principal/District
Supervisor that the teaching techniques/strategy was originally
developed by the applicant.
2.3 One copy of the certificate of Award/Recognition duly signed
by the Schools Division Superintendent.
2.4 Literature/Write-Up of the techniques/strategy originally
developed by the teacher.
3. Simplification of work that results in cost Reduction. 3.1 Brief project propposal
3.2 One copy of certification of the School Head/ District
Supervisor that the work plan of the applicant was
originally developed and has been tried out and found
effective and it resulted to cost reduction.
3.3One copy of the division memorandum indorsing the
work.
3.4 One copy of the certificate of Award/Recognition duly
signed by the Schools Division Superintendent.
4. Worthwhile income generating Project for pupils/students
given recognition by Higher Officials in the Division
4.1 Brief project proposal
4.2 one xerox copy of Certificate of Award/Recognition
duly signed by the Schools School Division
Superintendent
4.3 one copy of certification by the District
Supervisor/SDS that the project is originally developed by
the teacher.
B. Served as . . . . 12 points
1. Subject Coordinators/Grade Chairman for at least one year
2. Adviser of School Publication Dramatic Club and all other
pupil-related clubs as well as Teacher – related school
activities
And discharged such assignment satisfactorily for at
least two years and not considered part of the regular
teaching load
1.1 Special Order signed by the Principal and the District
Supervisor as such
1.2Duly approved Action Plan for 2 years
1.3.Narrative Report of Accomplishment for 2 years with pictures ,
duly signed by the head of school
1.4 Certificate of Recognition signed by the School Head
C. Served as Chairman of Special Committee such as : (12
points)
 Curriculum Study Committee
 Committee to prepare Instructional Materials/Test Materials
 Testing Committee
 Committee to prepare School Program (schedule of classes,
teachers assignment, and other teacher job – related/DECS
activities
1.1 Special Order signed by the School Head/ District
Supervisor designating the Teacher as Chairman of the
Special Committee for two years
1.2 Duly approved Action Plan for two years
1.3 Narrative Report on the Accomplishment made within the
two years as Chairman, with pictures or sample with the
name of the propoenent
1.4 Original Copy or sample of the activity done/ (1 copy)
prepared by the teacher and concurred by the head.
1.5 Certificate of Recognition signed by the head of school or
District
D. Initiated or headed Educational Research duly approved
by theSchools Division Superintendent or Regional
Director related to Community development or teachers’
Welfare or BERF approved Research Topics. . . . . . . 12

intent/permission to conduct.The proposal must be approved
first before the conduct of the AR)
E. Coordinator of community project/activity of
another agency or rural service improvement activity
such as feeding, nutrition, agro-industrial fair for at

1. Letter requesting the teacher to conduct the activity/or lette r of
intent of the teacher to conduct the activity;
2. Special Order signed by the school head
3. Project Proposal/Term Event Request with Budget and
Training Design
4. Narrative Report on the conduct of the Activity with pictures
,handouts ,outputs and other necessary documents to prove of its conduct
(Rapporteur’s Report, Evaluation of the activity
5. Activity Completion Report
6. Certificate of Recognition signed by the School Head

Worthwhile income generating Project for pupils/students given recognition by Higher Officials in the Division 4.1 Brief project proposal 4.2 one xerox copy of Certificate of Award/Recognition duly signed by the Schools School Division Superintendent 4.3 one copy of certification by the District Supervisor/SDS that the project is originally developed by the teacher.
