Transitions between child and adult services can be problematic due to differences in priorities, eligibility criteria, budgets, culture and staff. Young people are often not involved in transition planning. Integration of health and social care services aims to improve outcomes, but faces challenges from differences in organizations and professions. While early evidence from Scotland shows integration improving communications and flexibility, concerns remain that it could increase bureaucracy or fragment services for some. Successful integration requires involvement of individuals, local focus, and consideration of each person's situation.
