- 4. Step1. First you look for the medisoft icon on the computer Step2. Double click on the medisoft icon Step3.Once an chart pops up, you are ready to put a patients info. Step4.On the Patient List chart there is a button with a plus sign symbol on it Step5. this button opens the Patients/Guarantor dialog box Once an chart pops up, you are ready to put a patients info. Step6. the dialog box has three tabs they are: Name, Address, other information and payment Plan tabs Tab1 Name/Address: this option is to obtain your basic info. About the patient: Name,Address,email,birthday ,SSI,Race,Language.the most important thing is the chart number you need that to find your history Tab2 Other Information: This tab gives you a option to gives the provider more information about yourself. Procedure codes or relating to coding Right below that is a ICD-9 is used to classify diseases and a wide variety of signs symptoms and questionable findings of the patients history The CPT code section is used to describe medical, surgical,raidology, management services physicians and health care providers. This form is broken down in three section. the first part is basic info.
