Designing Questionnaires
A step-by-step guide
Plan first
Scope out the background of the area of interest; establish a clear
rationale for the questionnaire; think about the main things you want to
find out; make sure you consider GDPR and confidentiality issues.

Consider question types
What work for your approach? Explore 'closed' questions (Dichotomous
responses; Multiple choice; Scale questions e.g.
Likert), and 'open' questions (free-text). How will you collect data -
online, paper-based?

Write the introduction
State what you are doing and why; engage the participant; create the
need for their response; be interesting!3
Initial (early) questions
Simple introductory (but important) questions e.g. age, gender,
ethnicity, questions about experience and interests. This sets the tone
of the survey and draws respondents in.

Main section
Focus on the big questions you really need answering - these will help
you meet your research aims.5
Thanks
Remember to include a thank you to the respondent together with a
contact email for queries.7
Final questions
Use this part to get additional important information that will help
interrogate the data e.g. job role, years experience, number of
sessions undertaken, etc., etc.
