 When a student turns 16 and has been in special
education services a meeting must be held by
the IEP team to discuss future services.This is
calledTransition (ITP).
 The ITP helps set goals for the student and helps
prepare them for adulthood.
 The ITP looks at the student’s interests, needs,
and preferences.
 If the student will need services in after public
schooling it is important that the student and
parents begin researching the services that will
be necessary.
 Age of Majority Statement- This is statement that will be
signed 1 year prior to the student turning 18.The
statements goes over the student’s rights for when they
turn 18 and talks about how they will then have the right
to approve or deny the IEP set up for them.
 Transition (IEP Goals)-This is the part of the transition
that will look at future goals for the student. For example,
employment options, adult service options, and
independent living options.
 Summary of Performances (SOP)- Once a student
graduates from high school or ages out the school must
provide them with ways to form a living after school life.
 Here are some examples of transition planning
programs:
 Workability-This program sets up internships for the
student so that they can learn how to be successfully
employed.
 Transition Partnership Project-This programs helps
students with service coordination, job coaching, and
job development.
 AdultTransition Programs- Students over 17 may use
this program to learn how to live within the
community and do so independently.
 Families will need to be educated on what
services their child will need once they leave
school and transition into adulthood.The
child will look for guidance from their families
and it will be important that their families are
there to help, guide, and support them in this
life change.
 It will be vital that the student practices being
self-determined so that they have to drive to
meet their goals without having others push
them.
 It is also vital for the student to practice self-
advocacy so that they can advocate for
themselves rather then having others do it for
them like in the past.
 Transition planning is very important. It helps
the student see what their future holds and it
helps their families prepare for the future.
Also, transition planning helps set goals for
the student to work towards and helps them
research what services will be available to
them after public school.
