This document provides a validation form for evaluators to assess the validity of Self-Instructional Materials (SIMs). The form contains sections to evaluate the Format and Design, Language and Style, Content, and Usability of the SIMs. The evaluator is asked to check boxes indicating whether each listed item meets standards of clarity, suitability, understandability, and ability to serve learners. They are also prompted to provide remarks on any items as needed. The validation process examines elements like readability, organization, language level, instruction clarity, activity design, assessment design, and ease of use of the SIMs.
