I am a teacher of Lamac National High School municipality of Pinamungajan. I would like to apply for an extension of my personal leave without pay from June 2012 until the month of August 2012. I have requested a personal leave last January 21 until March 31, 2012 due to family problem which is between me and my husband who is under the influence of drugs. I would like to extend my leave until I can transfer to my hometown which is in Tagbilaran City Bohol for my safety with my son. Hoping for you kind consideration in this special request.
