Google Forms
Learn how Google Forms allows you to create,
share, collaborate, and analyze forms and surveys
for free.

How to utilize Google Forms to create
forms and surveys?

Simple! You just need to have a Google
Mail account!

It is FREE!

Let’s get
started!

In your browser, type
gmail.com

Create an account, if you don’t have one yet.

If you already have one, input your credentials

Your email account

Click Next

Enter your password

Then Sign in

You will be directed to your home page

Click the upper right portion for the Google Apps:

Choose Drive

A new tab will appear with your
Google Drive details

Click NEW

Click Google Forms

An Untitled form will appear

Customize according to your requirements:

Rename the name of the form to the
name of the survey

Include Form Description

Type in your desired questions for your survey

Type in the possible choices

Click Add option to add more choices

Add more options as desired

You can also choose to add “OTHER” to create a blank
for your respondents to freely input their suggestion.

Click this area to choose the format how the
questions will be answered:

Tick the Required ﬁeld ON to make the question a
mandatory question

Click this icon to add a new question.

Notice ﬁrst that the ﬁrst question has an asterisk.

This means that this question is mandatory.

Now, type in your next question

Since your question requires a date, choose “date”
from the choices below:

Notice that the format of the answer ﬁeld is now
changed:

Turn the Required option ON

Let’s add one more question

Choose multiple choice so that only one answer can
only be chosen

Input the available choices

Make it a required question

Let’s have another question

Since this question requires free text, you can choose
from short answer or paragraph

Let us choose paragraph:

Notice that the answer ﬁeld shows “Long answer text”

For this one, let us not make it a required ﬁeld.

If you click add new question, you will see that the
previous question does not have the asterisk sign

This means that the respondents can opt not to answer
this question

Let us go back to this question. There is an option to
duplicate this same question.

To do this, click the duplicate icon below:

Clicking it will create a copy of the same question.

You can also delete a question, if not needed anymore.

Click the Delete Icon to perform this.

This notiﬁcation will show conﬁrming the deletion.

We are done with the questions!

Now, let us explore the other
functionality of Google Forms!

On the upper right hand portion, we can
change the theme of the survey form.

Click Color Palette to choose your
desired background

Choosing green will make your
background green!

Other than color palette, you can also
add a theme.

From the same color palette icon, choose
the last option.

All available themes will be displayed

Since our survey is about Team building,
let us choose from sports and games:

From Sports and Games, there are still
different options to choose from.

Choose one theme from
Sports and Games.

Then click Select

Voila! This is now how it looks like!

Now, let us see how the overall
survey form looks like.

Click the Preview button

A new tab will open, showing you how
the survey looks like

You can also try answering the questions
to test it.

Try leaving a required question blank.

Notice the notiﬁcation that this is a required
question that needs to be answered.

Complete the
survey

Then click Submit

It will prompt that your
response has been recorded.

Go back to your form and you will see there
is one item under “Responses”

Click Responses.

You will see the list of responses for this
survey.

You can view it through 2 options - summary
of individual.

Choosing Summary
will give you ALL
responses for each
question, in
graphical form.

Choosing Individual
will give you the
detailed answer of
each respondent.

You can also view the responses in a
spreadsheet.

Just click “Create Spreadsheet” to do this.

Choose your desired destination

Then click Create

Voila! You now have a report available!

It shows you the timestamp on when each
response was completed.

And all answers to each question, in a
spreadsheet format!

And you can also download this to your
computer.

Or you can just retain it in your Google
Drive, under Sheets

Let us go back again to the
Forms to check on some other
functionalities.

Click Settings

And customize based on your requirements

Then click Save, when done.

Lastly, to Send this survey to respondents,
click SEND.

You can send through email

Input all email addresses

Include a Subject for the Survey

And a message on what the survey is about.

You can also opt to include the form in the
email. Otherwise, a link will just appear.

You can also copy the link of the survey, and
send it separately to your respondents.

You can also choose to shorten the URL if
the original link is too long.

Click COPY

Lastly, you can also embed this to your
website

Just click copy and paste it to your website.

And we’re done! We have just completed a
Google Form and gone through the different
functionalities that it offers.
