Proposal Summary
The Smile Care Society seeks to provide information to troubled youth attending
Heritage Schools (a residential treatment center for troubled youth) about the effects that oral
piercings and tongue splitting have on the health of the mouth. The purpose of our study is to
show that oral piercings are local contributing factors to periodontal disease and systemic
infections. The results of the study will help the troubled youth refrain from, or properly care for
their oral piercings or tongue splitting.
Studies show that oral piercings negatively affect oral health including both local and
systemic problems. Local complications include damage to gums, teeth and fillings. While
systemic complications associated include various infections, Ludwig's angina, hypersensitivity
to metals, nerve damage, and possibly endocarditis. Oral piercings and tongue splitting are
occurring more and more because of the current trend in fashion. The troubled youth population
do not understand the risks involved with these trends.
The purpose of this program is to improve the overall knowledge of oral care to the
troubled youth population found at Heritage Schools. Our presentation will include a powerpoint
lecture informing the troubled youth of the negative local and systemic complications associated
with oral piercings. After hearing the lecture the troubled youth will be more aware of the effect
that an oral piercing can have on their body.

A. Organization Overview
The Smile Care Society is an organization which focuses on the correlation of oral
piercing and the overall health of the oral cavity. The Smile Care Society are practicing dental
hygienists who graduated from Utah College of Dental Hygiene in 2015. All three hygienists live
and work in private practices in Utah. The goal of our organization is to improve the oral health
of troubled youth concerning oral piercings and tongue splitting, and to inform them the negative
effects, and how to properly care for oral piercings and tongue splitting. The mission of our
program “To improve the oral health knowledge concerning oral piercings and tongue splitting
among troubled youth populations at Heritage Schools.” is to improve the knowledge
specifically regarding oral tongue piercing and tongue splitting, the effects they present on the
oral cavity, and preventive care in order to improve the oral health status of the people who have
oral piercings and tongue splitting.
B. Description of Programs/Program Goals and Objective
Program goals and objectives
-Increase knowledge about the oral piercings and tongue splitting in troubled
youth students residing in Provo, Utah
-Increase understanding of troubled youth students residing at Heritage School in
Provo, Utah of the relationship between health and possible diseases.
-Provide a lesson to the students to increase their knowledge of the negative
effects that oral and facial piercings and tongue splitting can have on their oral health.
Activity 1: The students will watch a 20 minute powerpoint lecture quickly teaching them the
negative effects or oral piercings and tongue splitting.
Activity 2: Have the students participate in a powerpoint matching game reiterating the negative
effects.
Activity 3: We will demonstrate the proper techniques for basic oral home care as well as correct
care and maintenance of oral piercings and tongue splitting. Then we will have each participant
repeat after our example to show that they have comprehended and retained the information.
Heritage Schools will provide a classroom with desks, chairs, and the technology to
present a digital power point. The hygienists will bring a flash drive with the powerpoint, and
will connect it to a computer provided by the staff. The presentation will be held in during the
students health class. Two presentations will be given one to the female’s class and another to
the male’s class.
C. Population Groups Served/ Needs Assessment
The purpose of this proposal is to increase the oral health knowledge of youth with oral
piercings and tongue splitting at Heritage Schools. Studies have shown a significant increase in
oral diseases with those that have oral piercings or tongue splitting compared to those who do
not.
Our target population focuses on youth with oral piercings and tongue splitting. 60% of
teenagers have a piercing, mostly girls, aged between 15 and 19 years . However, oral piercing is
especially prevalent in the male population (Inchingolo 2011). Many people are unaware of the
effects that oral piercings and tongue splitting can have on the overall health of their mouth. In
fact, many do not think there is any association with oral piercings or tongue splitting and the
health of the mouth at all. Our program will address the importance of basic oral home care, the
effects of oral piercings and tongue splitting on the health of the mouth, as well as, the proper
care and maintenance of oral piercings and tongue splitting.
Our resources will include a computer and projector which will be provided for us by
Heritage Schools, and an oral home care kit with toothbrushes, toothpaste, mouthrinse, and floss
for each of our participants. Other resources include: paper and printing for the surveys for each
participant and gas for traveling to and from Heritage Schools.
Our Program is different due to the fact that we will be going to the facilities where the
participants reside to present the program rather than having the participants come to our facility
for the program presentation. We will have a fun, motivating and interactive presentation which
will encourage participant involvement. We will demonstrate the proper techniques for basic oral
home care as well as correct care and maintenance of oral piercings and tongue splitting. We will
then have the participants demonstrate back to us what we presented to make sure they fully
understand the proper techniques so that they may be successful with their oral care at home.
D. Program Description/ Methods or Designs
Our program will be based off of a prior presentation given to a similar target population.
This project proved to be successful and is prospective to be just as successful to a larger
population. With the help of Ben Parker at Heritage Schools, we were able to organize a time
and place to present the information about the effects that oral piercings and tongue splitting
have on the oral cavity. On December 10, 2014 we will travel to Provo, Utah in order to present
the program to the troubled youth at Heritage Schools. Thirty youth will participate in the
presentation. Each participant will complete a survey prior to and at the end of the presentation
in order to assess the knowledge of oral piercings and tongue splitting of the participants. The
effectiveness of the program presentation and the participants’ knowledge about oral piercings
and tongue splitting and their effect on the oral cavity was evaluated. The evaluation was
administered by three dental hygienists. The introduction and explanation will be given by
Bethany Hogan, followed by the effects from oral piercings and tongue splitting given by
Ashlyanna Ogden, and the conclusion given by Amy Francis about proper management of oral
piercings and tongue splitting. The planning of our presentation was geared toward maintaining
the attention of the target population, as well as motivating them to incorporate the proper
techniques that we will be demonstrating into their oral home care routine.
E. Program Evaluation:
The program evaluation was conducted on December 15, 2014. The evaluation was
conducted using information gathered from comparison and contrast of the pre and post-tests.
This pilot study mirrored the presentation that will be used for our proposed “Oral Piercings and
Tongue Splitting Effects on Oral Cavity” program. Upon reflection we determined the following
information. The structure of the program was logical and well received. The majority of
participants were actively engaged during the entire presentation and activities. The most
participation was during the activity matching game. The success was due to the fact that we
made sure to adapt the activity to the participants’ interests as we went along while giving hands
on assistance.
The success of the program objectives was evaluated by comparing the responses on the
pretest and the post-test. The pre-test was administered prior to the presentation and the posttest
was given following the powerpoint and matching activity. The results showed that the average
scores of the pretest and posttest generally improved. The knowledge of oral piercings and
tongue splitting and their effects on the oral cavity increased, and the value placed upon daily
oral hygiene increased. The average of the pre-test was sixty percent compared to the average
posttest score of eighty-five percent.
These results show that our pilot program was successful. We correctly determined needs
for our target population and developed an effective program. Every member of our group
contributed to the pilot study. We were able to combine our strengths into an effective effort to
improve the oral health in a transitional population.

Toothbrushes: $90.00
Toothpaste: $120.00
Floss: $50.00
Mouthrinse: $120.00
Salary: $34.00 each per presentation. (max 3 presentations)
Totaling $686.00 all together including salary.
