The midyear survey is part of the developmental evaluation process, focused on surfacing
insights about what it takes to do the work of BUILD effectively on the ground. The learning
from the survey will be shared back with all participants and will be used to strengthen the
support services that are part of the BUILD approach.
Participation will be requested from the three core organizational partners - the non-profit
lead, public health, and the health systems partner - to increase the perspectives and value
of the information generated.
Participation from sites is required and answers will be anonymous.
Detailed analysis of
survey data will inform:
• the development of
Learning Sessions
• current operations of
BUILD
• the development of
future iterations of
BUILD
No more than 25
minutes for the
non-profit lead.
No more than 10
minutes for the
public health and
health systems
partners	
  
One-time, online
survey open in
December 2015.
Hospital Engagement
Project

The Hospital Engagement Project is designed to achieve two distinct goals:
• Explore hospital and health system motivation for partnering in the BUILD Health
Challenge
• Examine the definitions and approaches to addressing the social determinants of
health, population health, and health equity most commonly used by hospitals and
health systems
Data from the mid-year survey will inform this project. Additional participation will be
requested from the hospital or health system partner organization. Additional references for
interviews within the health system may be requested.
Participation is voluntary but strongly encouraged.
Key insights on health

be published in a peer-
reviewed journal and
other relevant channels.
A one-hour
interview with the
primary
hospital/health
system partner on
the BUILD project
team.
A one-hour
interview with an
additional contact
at the
hospital/health
system partner
(preferably the
hospital/health
system CEO).
BUILD Case Studies The purpose of the BUILD case studies is to document how you and your partners worked
together over the course of the funding period. These case studies will document the “how”
of BUILD; this could include topics such as how your collaboration:
• collectively addressed the social determinants of health
• collaborated to address a particular upstream issue

Case studies will be
profiled on
buildhealthchallenge.org,
as well as featured in a
series of papers aimed
at documenting best
practices in multi-sector
collaboration.
Interviews will be
conducted with
each of the three
core partners
every other month.
Interviews will take
one hour.
