The Columbia Accident Investigation Board (2003) generated 248 pages -- neither were able to bring back the lives of the people who lost their lives. Both pointed to issues that had been documented – and ignored repeatedly.Agile isn’t about lack of documentation – it’s about lack of ceremony in documentation.
